ADHD medication rules in India: why only a doctor can prescribe
How ADHD medication works legally in India. Schedule X rules for stimulants, who can prescribe, why the pharmacy keeps your prescription.

The short version
Every ADHD medication in India needs a prescription from a registered medical doctor. The most common stimulant, methylphenidate, sits in Schedule X of the Drugs and Cosmetics Rules, the strictest retail schedule ordinary medicines reach.
That one fact explains most of the confusing stuff: why the psychologist who assessed you sends you to a psychiatrist for the prescription, why the pharmacy keeps your prescription paper, and why “just order it online” is both illegal and a bad idea.
Who can prescribe
Prescribing in India is reserved for doctors on the NMC or state medical council registers. For ADHD, in practice:
- Psychiatrists. The usual prescribers, adult and child.
- Developmental paediatricians. Routinely prescribe for children as part of a wider plan.
- Psychologists, counsellors, therapists: no. Central to assessment and therapy, yes. Legally able to prescribe, no. Someone without a medical registration offering to “adjust your dose” is a red flag, full stop.
This is why care here often runs as a two-person setup. Psychologist for testing and therapy. Psychiatrist for the diagnosis confirmation and the prescription. Neither replaces the other, and the good ones work together without you having to push.
What Schedule X means for your Tuesday
The rules live with the CDSCO, and the part that touches your life is retail. For a Schedule X medicine:
- The pharmacy needs your original prescription and keeps it. Ask your doctor how repeats will work before you leave the clinic, not at the counter.
- Not every pharmacy stocks it. Licences for Schedule X stock are separate. Your clinic will know which nearby pharmacies carry it.
- Repeats need fresh prescriptions. This is why follow-up appointments are built into stimulant treatment. It’s the system working, not an upsell.
- Some ADHD medicines, like atomoxetine, sit in the normal prescription schedules. Still prescription-only, but without the keep-the-paper step.
What good monitoring looks like
A prescription is the start, not the end. The standard most clinicians follow (NICE NG87 is the common reference) expects doses to be built up gradually with regular reviews: how you respond, side effects, blood pressure and pulse, and for children, height and weight tracked over time.
Concretely: frequent reviews every few weeks while finding the dose, then routine check-ins once stable. If someone hands you a stimulant prescription with no follow-up plan at all, that tells you something, and not something good. Three things that are never okay
- Buying stimulants without a prescription. The seller is committing a crime and the product is unverified.
- Taking someone else’s medication. “Just try my tablet” skips every safety check prescribing exists to do.

India’s rules make ADHD medication mildly inconvenient on purpose. The inconvenience is the safety mechanism. Work inside it with a verified doctor and it’s entirely manageable.
Frequently asked questions
Can a psychologist or counsellor prescribe ADHD medication in India?
No. Prescribing is legally a registered medical doctor's act. A clinical psychologist, however well trained, cannot write you a prescription. In practice the prescriber is a psychiatrist or paediatrician, often working alongside a psychologist who handles testing and therapy.
Why does the pharmacy keep my methylphenidate prescription?
Because methylphenidate is a Schedule X drug under the Drugs and Cosmetics Rules. The pharmacy can only dispense it against a valid prescription from a registered doctor, and the law requires them to keep the prescription copy and maintain records. The pharmacist isn't being difficult. That's the rule.
Can I buy ADHD medication online in India?
Not legally, for stimulants. Schedule X's keep-the-prescription rule rules out casual online orders. Any site shipping controlled stimulants without a prescription is breaking the law, and you have no idea what's actually in the strip they send. Non-stimulants still need a doctor's prescription too.
Is medication compulsory after an ADHD diagnosis?
No. It's one option. Saying no doesn't cancel your diagnosis or end your care. Therapy and practical strategies stand on their own. That said, many people do end up including medication in the plan because its effect on core symptoms is large.
